Social services in Conca de Barberà are strengthening their home care model, a trend that is consolidating due to the progressive aging of the population and the increase in assistance needs. This commitment aims to ensure that people can remain in their homes with autonomy and security.
The Home Care Service (SAD), managed by the Fundació Vilaniu, has recorded a considerable increase. During 2025, 75 active services were accounted for, with a 22% increase in the volume of hours dedicated to dependent individuals, exceeding 6,800 hours of care. Additionally, hours related to social care reached 4,844, demonstrating the growing demand for support for family caregivers.
Another key indicator is the increase in dependency assessment requests, with 1,160 petitions processed in 2025, a 16% rise from the previous year. These assessments are fundamental for accessing public system benefits and resources. Regarding case management, Individual Care Plans (PIA) continue to be the foundation, with an increase in follow-ups to 387, ensuring more personalized support.
This comprehensive model is complemented by other essential services. Telecare has consolidated as a vital tool, with 78 users having a permanent alert system. Adapted transport has facilitated access to day centers and specialized resources for 27 people. Finally, the Technical Aids Bank has provided support material, such as articulated beds or walkers, to 140 individuals, improving their autonomy at home.
