The Home Care Program for Hospital Discharge (PADAH) has received the second prize in the We Move for People category. This program facilitates a safe transition from hospital to home for individuals with a loss of autonomy, promoting functional recovery and enabling them to remain at home. Since its inception ten years ago, it has assisted over 1,200 people.
The other awarded project is the Comprehensive Care Platform for Very High Vascular Risk Patients (PAI-HF), which also secured a second prize, this time in the We Innovate to Transform category. This platform incorporates artificial intelligence for screening, risk stratification, and clinical decision support to better identify patients, improve therapeutic goal achievement, and prevent cardiovascular events.
In this first edition of the Community of Good Practices Awards, a total of 209 candidacies from 49 entities were received, with 9 initiatives being recognized.
The 7th +FUTUR Conference, under the theme Authenticity to Lead, Leadership to Transform, brought together over 450 professionals and executives to address decisions shaping this transformation, including overcoming care fragmentation, adapting organizational models, incorporating technology, responding to professional shortages, and advancing in health and social integration.
