The initiative, already benefiting about ten users of the EAP Bàscara, affiliated with the Catalan Health Institute (ICS) in Girona, aims to speed up the location of isolated dwellings, such as farmhouses, where conventional addresses are insufficient.
This information can be consulted directly from the Primary Care Clinical Station (ECAP), the computerized clinical history system. This helps reduce location time, improve professional safety, and expedite emergency responses, particularly for patients in the Home Care Program (ATDOM).
Healthcare professionals record the exact access point to the dwelling using Google Maps during home visits. The validated information is incorporated into the ECAP, transitioning from verbal directions to precise geographical references.
The project has evolved with the generation of QR codes, which instantly open the location on a mobile device, further simplifying information access and navigation, and facilitating interconnection with other healthcare services.
Key results include increased accuracy in locating homes, better coordination among professionals, and more efficient healthcare response, ensuring equitable access to health services regardless of residence.
The EAP Bàscara considers the initiative simple, sustainable, and replicable in other areas with similar population dispersion.
This initiative was recognized during the 14th Scientific Conference organized by the Fundació Salut Empordà, highlighting the adaptability of primary care.
