More than 200 health and social rights professionals gathered Friday at the Sixto Mir Auditorium in La Ràpita to hold the 1st Chronic Care Conference Terres de l’Ebre, aiming to improve integrated care for chronic patients.
The meeting, held for the first time in the Terres de l’Ebre region, aimed to share experiences, best practices, and strategies to improve care for people with chronic diseases, prioritizing an integrated and person-centered approach.

The Health delegate in Terres de l’Ebre, Ferran Roche, stressed that increased life expectancy implies living with more chronic diseases, which is one of the major current health challenges. A guidance document for addressing chronicity, developed with the participation of professionals from the territory, was presented during the conference.

The Territorial Director of Social Rights in Terres de l’Ebre, Javier Sánchez, highlighted the importance of coordination between the health and social spheres. Sánchez noted that, despite the difficulties arising from demographics and territorial dispersion, professional collaboration is a strong point of the region. The program included presentations and round tables on integrated care, interdisciplinary work, and future challenges.
