EAP SIVIMAR reduces 86% of in-person visits for medication renewal with innovative project
The new system centralizes the management of medication plans in administrative staff, freeing up care time for medical professionals.

The Primary Care Team of Sils – Vidreres – Maçanet (EAP SIVIMAR) has managed to reduce in-person visits to the CAP for medication renewal by 86%, thanks to a project launched last March that centralizes plan control in administrative staff.
This innovative project is based on the proactive review of medication plans. Every month, the Information and Communication Technologies Unit of the IAS-ICS Girona Primary Care and Community Management extracts a list of patients whose medication expires within the next 60 days.
Administrative staff classify cases into two circuits: the Planned circuit, for chronic patients with pre-proposed actions, and the Unplanned circuit, for long-term treatments not requiring a visit. If a medical visit is necessary, the administrator proactively schedules it and notifies patients via SMS.

The initial results are conclusive: out of 997 patients with active plans monitored, 856 receive medication without needing a visit. Only 85 patients require a face-to-face appointment with their primary care physician.

Given the success, the program will be extended in the autumn to the EAPs of Cassà de la Selva and Salt, with the main goal of implementing this experience across most basic health areas of the Girona Health Region in the coming months.
