Gandesa to Lead New Integrated Health Model in Terres de l'Ebre

The Gandesa Primary Care Center (CAP) has been designated as the hub for the Integrated Health Center (CSIR) model in the Terres de l'Ebre region, focusing on the Terra Alta area.

This strategic move aims to provide healthcare professionals with greater autonomy and better management tools. The Terra Alta health area serves approximately 11,500 residents across twelve municipalities, making it an ideal location to test solutions for rural healthcare challenges.

A key feature of the project is a telemonitoring pilot program for home care. This technology will allow doctors to remotely track the vital signs of chronic patients, reducing the need for travel in a region characterized by long distances and an aging population.
The ICS Terres de l'Ebre expects this initiative to become a benchmark for other rural areas in Catalonia. An evaluation system will be implemented in the coming weeks to measure the impact on patient satisfaction and clinical efficiency.
